Prepared for Kevin Quick, Del City, Oklahoma, citing birth record and noting "The birth index was also examined for Agata Coloaquila. From this examination, it was discovered that Coloaquila is not the correct spelling of Agata‘s surname. There were no entries in the index for this surname. This prompted a surname study which showed that Coloaquila is not a valid surname. Rather, “Coloaquila” was most likely a misspelling of the surname “Colaiacovo"—a fairly conmmon surname in Pratola Peligna. There was just one entry for an Agata Colaiacovo in the index. This Agata was born on 24 October 1856 to Nicola Coloiacovo, age 30, and Vincenza Palombiaio, age 23. Since this was the only Agata Coloiacovo in the index, it was very likely that this was Santo’s mother, but more information was needed to say this for certain."
